Terms and Conditions

To qualify for the Dental Cooperative negotiated Over $1MM Tier with Alphaeon Credit, participants must meet the following criteria:

1. Participants must be a current member of the Dental Cooperative in good standing.

2. All associated items subject to the negotiated provider fees must be purchased through the identified sales channels as specific under Alphaeon Credit Terms and Conditions.

3. Tier One Provider Fee structure begins only after agreement has been received and processed by the Dental Cooperative and Alphaeon Credit.

4. Upon termination from the Dental Cooperative, Member will no longer have access to Alphaeon’s Over $1MM Tier defined in the terms.

5. Alphaeon Credit can share all patient application and funding statistic data with the Dental Cooperative, but not patient information under HIPAA Privacy Rules.

By proceeding with enrollment in the Aphaeon Credit, I agree to the Dental Cooperative Terms and Conditions.         

*Provider Fees are discounted based on projected annual financing volume. Patients select from the plans you choose to offer. For charges under $250, the Revolving Practice Fee is 3.5% for practices financing under $500K annually, 3.3% for practices financing $500K to $1MM annually, and 3.15% for practices financing over $1MM annually.
